Facial shape is influenced by three factors: Skin, fat beneath the skin, and bone structure.
The amount of facial fat genetically varies among people. Some have full faces; some have thin faces. With aging or weight loss, facial fat may decrease, leaving a gaunt, hollow appearance.
Facial volume may be restored either with injectables ( Restylane, Perlane, Juvaderm, Sculptra, Radiesse), these fillers are less practical for filling large areas.
Fat cells harvested from the abdomen or thighs can be re-njected inot the face to permanently restore volume. The advantage of fat is that it is the patient’s own normal tissue – you are replacing what’s missing!
The main disadvantage to fat as a filler is that fat cells are very fragile. When harvested and re-injected, only 30-50% of the fat cells survive. This unpredictability may lead to the need for re-injection. In addition, the procedure must be done under sterile conditions.
Fat injection is useful in the temples, orbits, cheeks, and chin (“marionette lines”).
